The salient points in the clinical history were as follows The patient first noticed a small lump in the neck when he was twenty-two years of age; for the next two years this grew rapidly, and then for thirteen years remained quiescent; that is, up to the time of his death._ The tumor had never caused any pain or discomfort. Before his death the diagnosis of calcified tuberculous glands was made, the mass being a little too high to be considered a cystic or calcified lobe of the thy; roid.

The attempts to discover the cause of malarial disease are of considerable historical interest. From earliest times the existence of a contagium vivum has been generally assumed, by reason of the peculiar clinical features of the disease itself. Up to 1879 most of the hypotheses that were offered assumed that some one of the fresh-water alga was the exciting cause.l These observations were never confirmed, and obtained but little belief.

Bastian cannot be accepted. The degeneration of the nerve fibres in the posterior root-zone of Flechsig no doubt explains the absence of the reflexes in these cases. In the alcoholic polyneuritis, the changes in the nerves could in themselves be sufficient to cause the loss of the reflexes, but the degenerations in the nerves are not so marked as the degenerations in the posterior root-zone of Flechsig.

The specimen was removed from a man who had suffered for a number of years from pulmonary phthisis. III the small intes tine there were several small tubercular ulcers, while the colon presented a number of ulcers with irregular margins and smooth bases. Microscopical examination of these ulcers showed some remnants of tubercular disease, but their bases were covered with new connective tissue. It was certainly rare to find a case in which tubercular ulcers of the intestines were healing.

Dr. Ewing said that about six months before he had had occasion to bring together some evidence regarding the nature Of vaccine bodies. This evidence pointed to the view that red blood cells sometimes assumed the position and exhibited the staining qualities Of vaccine bodies. He had not claimed that all vaccine bodies were of this nature, nor did he wish to go so far yet. He merely wished to present some evidence which he had Obtained in favor Of this theory. The hemosiderin re action he had not been able to get in every instance; in fact, in the majority Of cases the stain had been a failure, and in some sections he had succeeded in getting the blue stain in only a portion Of the vaccine bodies. The specimens were hardened in bichloride saturated in 6% salt solution, cut in paraffin and allowed to remain twenty-four hours in potassium ferrocyanide, and six hours in hydrochloric acid. The blue color was seen only in the center Of the vaccine body and by staining with eosin or safranin the outer portion Of the body took on a red stain. In many sections Of human tissues the159 james ewing, M.D., and israel strauss, M.D.

While under observation the essential features which were noted clinically were pigmentation (to be described in the au topsy protocol), marked asthenia, weak pulse (b. P. 84 mm Hg), and signs of apical tuberculosis. She had an antemortem rise of temperature to E, and died with pulmonary edema.

From this date, in all subsequent examinations, glucose was present in amounts varying from a trace to 994 grams in 94 hours. The specific gravity ranged from to averaging about Total twenty-four hour amount never exceeded four litres. NO examina tion for acetone or diacetic acid is recorded. For a time in the Fall Of 1903, and the Spring Of 1904, the patient received Parke Davis's pituitary extract, without Obvious benefit, although the patient said that he felt better.

During life, the patient had a marked stooping carriage. Death occurred on January 19. Patient had had a series Of severe convulsions, followed by a coma and cheyne-stokes respiration.

Summary Of Clinical History - Total duration Of illness, June, 1909, to January, 1907, - 4% years.

Principal Symptoms: Progressive enlargement Of jaw, nose, ears, hands and feet. Persistent frontal head aches, and attacks of vertigo. Gradually increasing loss of vision, ending in double Optic nerve atrophy and practically complete blindness. Recurrent epileptiform attacks. Glycosuria for twenty-two months.

For the present purpose, only the significant features Of the post-mortem findings need be given.

The body was that of a large, well-developed and well-nourished male, about kilos in weight. Length Of body on table, 5 ft. 8% inches.
