Описание: This book provides a comprehensive overview for those interested in research and promoting sexual health in older people, as well as a “go-to” guide on the topic of sexual behaviour in older adults. Sexual activity is an essential part of intimate relationships, though it tends to decline in frequency as people grow older. Nevertheless, for many older people, sex still remains an important part of their life. There’s a common misconception that as people age, they lose their interest in sex and capacity for sexual behaviour. This is not the case, as a number of national surveys show. The new research presented here indicates that the less sex older people have, the more likely they are to experience mental and physical health problems. Men and women who reported a decrease in the frequency of sexual activities were also more likely to report a deterioration in self-perceived health. Additionally, men with erectile dysfunction were also more likely to be diagnosed with cancer or coronary heart disease. This research has also determined that older adults who experience a decline in sexual activity report poorer well-being than those who maintain their levels of sexual desire, activity and function in later life, and that men who remain sexually active in later life continue to have better cognitive performance compared to those who don’t. Despite these health benefits, medical professionals do little to promote sexual activity in older adults, and the literature suggests that there is a lack of knowledge on how to approach the subject and how to promote such activities. This is the first comprehensive book specifically exploring all areas of sexual behaviour and health in older adults, and drawing on the latest research in this area. It offers a valuable resource for researchers and clinicians in various health fields (medicine, nursing, occupational therapy, psychology, public health, etc.), as well as students in undergraduate and graduate programs.

Описание:

"This is one of the best mental health and aging books I have ever read. It] is one that I will turn to often in my teaching of doctoral students, and in my work with older adults. One of the phenomenal aspects of this book is the research reviews; which are in-depth and broad in their scope. It is clear that Lee Hyer is an exceptional scholar-clinician and geropsychologist.."--Peter A. Lichtenberg, PhD

Drawing from current research and clinical practice, this text espouses a unique interdisciplinary approach to the assessment and treatment of psychosocial impairment in older adults. This approach, called "Watch and Wait," is grounded in a "whole person" model of care rather than one that addresses symptoms or syndromes in isolation. This model advocates relationship building, prevention, psychoeducation, multipronged interventions for comorbid problems, and communication. It does so in the context of a multidisciplinary health care team, the patient, and family.

The model encompasses five core factors of psychosocial impairment in older adults: depression, anxiety, cognitive deficits, adjustment or life problems, and health issues. Considered together, they provide an integrated understanding of the older adult that is more likely than single-disorder models to facilitate successful outcomes for common psychosocial difficulties that occur in later life. The book describes in depth the unique components and complex interactive influences of these five factors as they apply to older adults seeking mental health treatment. A cornerstone of the author's approach is treatment based on stringent care-based assessment and thorough monitoring of empirically supported interventions. Each factor is considered individually from its empirically supported basis as well as its interaction with the other factors. Distinct treatment modules are isolated for each factor and assembled to provide the optimal pathway for clinical treatment.

The text also addresses the unique difficulties of diagnosing the aging population, the pitfalls of existing treatments, and the need for brain-based models for care. Key Features: Advocates for integrative, interdisciplinary care and primary care involvement for the older adult Emphasizes core components of care: depression, anxiety, cognition, pain/sleep/health issues, adjustment in the community Demonstrates how a single-problem approach for older adults with psychiatric problems is not effective or efficient care Espouses a "Watch and Wait" paradigm of care, based on person-centered diagnosis and careful monitoring of treatment Translates and integrates current research findings with clarity Covers use of SSRIs and other medications, suicide, subsyndromal states, issues involving cost of care for the older patient, among other topics Plentiful case examples

Описание: Polycystic ovary syndrome (PCOS) can cause a wide range of symptoms, both physical and psychological. This book combines cutting-edge psychological and social science research with compelling personal stories to provide a roadmap for researchers and mental health providers.

PCOS can cause menstrual irregularities, infertility, insulin resistance, excess and male patterned body hair, and is strongly associated with depression and anxiety.

PCOS affects everything from one’s self-conception and identity, to social relationships, intimacy and sexuality, to physical and mental health. Yet the literature has been largely silent regarding the personal and psychosocial experiences of those with PCOS.

Using clear and compelling language, the author promotes a wider understanding of PCOS, calls attention to areas of research need, offers insight to mental health providers whose clients suffer from PCOS, and aims to inspire more positive outcomes for individuals with PCOS.

Описание: Meeting the Needs of Older Adults with Serious Illness: Challenges and Opportunities in the Age of Health Care Reform provides an introduction to the principles of palliative care; describes current models of delivering palliative care across care settings, and examines opportunities in the setting of healthcare policy reform for palliative care to improve outcomes for patients, families and healthcare institutions. The United States is currently facing a crisis in health care marked by unsustainable spending and quality that is poor relative to international benchmarks. Yet this is also a critical time of opportunity. Because of its focus on quality of care, the Affordable Care Act is poised to expand access to palliative care services for the sickest, most vulnerable, and therefore most costly, 5% of patients- a small group who nonetheless drive about 50% of all healthcare spending. Palliative care is specialized medical care for people with serious illnesses. It focuses on providing patients with relief from the symptoms, pain, and stress of a serious illness—whatever the diagnosis or stage of illness. The goal is to improve quality of life for both the patient and the family. Research has demonstrated palliative care’s positive impact on health care value. Patients (and family caregivers) receiving palliative care experience improved quality of life, better symptom management, lower rates of depression and anxiety, and improved survival. Because patient and family needs are met, crises are prevented, thereby directly reducing need for emergency department and hospital use and their associated costs. An epiphenomenon of better quality of care, the lower costs associated with palliative care have been observed in multiple studies. Meeting the Needs of Older Adults with Serious Illness: Challenges and Opportunities in the Age of Health Care Reform, a roadmap for effective policy and program design, brings together expert clinicians, researchers and policy leaders, who tackle key areas where real-world policy options to improve access to quality palliative care could have a substantial role in improving value.

Описание: Community-based integrated care (CBIC) is designed to keep older adults where they want to be – at home and out of hospitals and nursing homes. The Program of All-Inclusive Care for the Elderly (PACE) is an example of successful CBIC, established in 1985 as an innovative solution to pervasive issue: how to provide integrated care to a high-risk population of older adults that promotes community living (not a nursing home) in a cost-effective manner. This unique guide provides readers with a concise yet informative base of understanding of PACE as well as a “deep dive” into the components and job roles that will serve as a reference for existing PACE providers and will inform those who are considering the possibility of developing a new PACE program. Chapters are organized into five thematic sections. Part one presents both domestic and international perspectives on CBIC, with part two delving deeper into the US PACE model, including its history, policy evolution and effectiveness. Specifics regarding the organization and management of PACE and the essential roles and positions comprise parts three and four, respectively, from leadership and the involvement of the state to the roles of the social worker, nurse, nutritionist, rehabilitation specialist, and more. Care planning, coordination and end of life care round out part five. In addition to geriatricians and other clinical practitioners involved with the care of older patients, Community-Based Integrated Care for Older Adults is also a resource for scholars, policy makers and the general public who are interested in innovations in long-term care that provide older adults with alternatives to nursing homes.

Описание:

" This] book provides a practical hands-on perspective for implementing function focused care in all settings... It] is a unique resource that is relevant for all nurses and health care providers working with older adults. In addition to addressing functional decline, function-focused care provides a fresh and practical solution to many of the problems that tend to arise with older adults such as infections, falls, and pressure ulcers all known to be associated with immobility."

From the Foreword by Colin Milner
CEO, International Council on Active Aging

"This book encourages individualization, which is difficult to achieve with more prescriptive approaches. This second edition includes chapters on cognitively impaired adults, ethical issues, and patient-centered care, all of which are so important in long-term care. I found it to be very motivating in the way it helped me apply the philosophy and strategies to elders with whom I come in contact. I would highly recommend this book to anyone working with elders. Score: 95, 4 Stars--Doody's Medical Reviews

The purpose of restorative care nursing (often referred to as function-focused care) is to take an active role in helping older adults maintain their highest level of function, thereby preventing excess disability. This is the only volume to educate caregivers about both the philosophy of restorative care and how to integrate it into all care settings for older adults. Now in its second edition, the text contains updated content in each chapter along with two entirely new chapters on function-focused care for cognitively impaired adults, ethical issues, and patient-centered care.

The book provides a complete six-week education program in restorative care for nurses and other caregivers, numerous practical suggestions for beneficial activities that will enhance function, and strategies for motivating both older adults and caregivers to engage in restorative care. Woven throughout the text is research that documents the benefits and expected outcomes of restorative care. The book also includes the requirements for restorative care across all settings, and the necessary documentation. "Restorative Care Nursing for Older Adults" will help formal and informal caregivers and administrators at all levels assimilate the philosophy of restorative care and be able to develop and implement successful restorative care programs. This New Edition Features:

Completely updated information, including two new chapters on function-focused care for cognitively impaired adults, ethical issues, and patient-centered care A six-week education program that teaches practical application of restorative care nursing Helpful suggestions and strategies for motivating older adults and caregivers Education materials designed for nursing home, assisted living, and acute care settings, including required documentation and goal-setting forms Educational materials for family caregivers
