Описание: Preeclampsia is becoming an increasingly common diagnosis in the developed world and remains a high cause of maternal and fetal morbidity and mortality in the developing world. It is a multisystem progressive disorder of pregnancy characterized by the new onset of hypertension and proteinuria of ≥300 mg/day or of hypertension and significant end-organ dysfunction with or without proteinuria. It also presents with a variety of organ failures, including malfunction of kidneys, liver and lungs. It is estimated that about 5-7% of pregnancies are complicated by preeclampsia. This disorder always resolves after delivery. In about 80% of cases, pregnancy ends with good maternal and fetal outcomes. However, these pregnancies are still at increased risk for maternal and/or fetal mortality or serious morbidity. The remaining 20% of cases are at risk for preterm delivery and small gestational age infant. As regards the long-term consequences, women with preeclampsia are at high risk for cardiovascular, renal, and chronic hypertensive diseases. The exact pathophysiology of preeclampsia is poorly understood and, consequently, there are no well-established methods of primary prevention or of cost-effective screening. Childbirth is the only known cure; however, the decision between delivery and expectant management depends on several factors, such as fetal gestational age, maternal and fetal status at time of initial evaluation, presence of labor or rupture of fetal membranes, and level of available neonatal and maternal services. The aim of this book is to provide an overview of the latest developments about the physiopathology, diagnosis, and treatment of pre-eclampsia. Addressed to gynecologists, obstetricians, researchers and students, this text aims to become a reference for all operators who are interested in pregnancy complications and in the management and treatment of this specific disorder which is increasingly common in the population.

Описание: Uterine fibroids (UFs) are benign masses that develop from the smooth muscle cells and connective tissue of the wall of the uterus under the influence of genetic and / or hormonal stimuli. These benign tumours are postulated to arise from a single, genetically altered, mesenchymal cell under the influence of gonadal hormones namely progesterone and 17𝛽-estradiol. The annual societal cost for fibroids is estimated up to 34 billion dollars, calculated through combined expenditures for medical management of symptomatic fibroids, lost work attributable to diagnosis of fibroids, and obstetrical complications of fibroids in Italy, for example, they afflict 3 million women. Estimates say that 20 to 80% of women experience them throughout life, and are the most common form of benign cancer in childbearing age. Various surgical and medical options are currently available to manage symptomatic uterine fibroids. The choice of the appropriate therapeutic approach for UFs depends on several factors, including womens age, parity, childbearing aspirations and wish to preserve fertility, extent and severity of symptoms, size, number and location of myomas, risk of malignancy and proximity to menopause. However, this is not so for non-specialist physicians and for the women who suffer from these and for whom they are a true cause for alarm. Therefore, it is very important for physicians who are not specialists in cancer to know the signs and symptoms of each of these benign breast diseases in order for them to be able to emit a diagnostic impression and to suggest treatment. Описание: Endometriosis is a chronic benign estrogen-dependent disease which is characterized by the presence of endometriotic glands and stroma outside the uterine cavity. It affects women during reproductive age, but it may also be diagnosed in menopausal women. Endometriosis can be asymptomatic; but most frequently it can be responsible for pain symptoms and/or infertility, severely impacting women' quality of life.

Overall, clinical presentation depends on the location of endometriotic lesions: intestinal symptoms can occur in case of bowel nodules whereas urinary symptoms can occur in case of urinary tract endometriosis. At the moment, the exact prevalence of endometriosis is unknown because its definitive diagnosis requires surgery and histological evaluation. It has been estimated that this benign chronic disease affects at least 3.6% of reproductive age women; however, its prevalence could significantly increase considering women suffering from dysmenorrhea, chronic pelvic pain and/or infertility.

The etiology of endometriosis is unclear: various factors, such as multiple abnormalities of the immune system, genetic factors and environmental factors may influence women' susceptibility to develop this chronic disease. Although the gold standard for the diagnosis of endometriosis is the pathological evaluation of surgical biopsies or specimens, today transvaginal ultrasonography is largely employed to reliably identify the presence of deep infiltrating endometriosis or ovarian endometriomas. The aim of the surgery for endometriosis is to restore the normal anatomy by removing endometriotic lesions and by performing adequate removal of abdominal adhesions.

However, surgery, in case of deep infiltrating implants, can be rather challenging for gynecologists due to the complexity of pelvic adhesions involving several structures and it may be characterized by rare but not negligible urological, intestinal, neurological and vascular complications. Furthermore, pain may recur after conservative surgery for endometriosis. Although surgery is obviously required in cases of ureteral stenosis, bowel occlusion or ovarian cysts with doubtful characteristics of malignancy, medical therapy, and, particularly, hormonal therapies, are the initial therapeutic approach today for most patients with endometriosis-related pain.

Long-term medical therapy for endometriosis aims to minimize the production and action of estradiol, inhibiting the production of cyclic hormones from the ovaries. Numerous medical options (such as estroprogestins, progestins, gonadotropin- releasing hormones analogs and antagonists) are available for treating women with endometriosis: the choice of the most suitable compound is based on several factors, such as intensity of pain, age, desire to conceive, costs, route of administration and impact of the endometriosis on work capacity, sexual function and quality of life of each women. Currently, research is focused on finding new innovative targets to treat this benign chronic disease; in fact, it is well known that development, maintenance and progression of endometriosis depend on a variety of aberrant mechanisms including cell proliferation, immune function, apoptosis, invasion capacity and angiogenesis.

The aim of this book is to give an accurate overview on pathogenesis, diagnosis and therapy of endometriosis.
