Описание: This book is a derivative of Dr. Miller’s well-regarded Cerebral Palsy and focuses on rehabilitation techniques. The book discusses the theory and techniques used by physical and occupational therapists, and also provides suggestions for implementing an Individual Education Plan. Tables and algorithms are included to help in the decision-making process for determining what treatment would be most beneficial. Chapters in section one deal with fundamental aspects of cerebral palsy management, from understanding motor impairment, to evaluating equipment use, to navigating the relationship with the families of patients. In section two, physical therapists cover specific rehabilitation techniques, including neurodevelopment therapy, hippotherapy, and assistive devices. On the accompanying CD there are more than 100 case studies, as well as videos of children with gait disorders.

Описание: The book has been designed to provide readers with an understanding of cerebral palsy (CP) as a developmental as well as a neurological condition. It details the nature of CP, its causes and its clinical manifestations. Using clear, accessible language (supported by an extensive glossary) the authors have blended current science with metaphor both to explain the biomedical underpinnings of CP and to share their awareness that there is much that can be done to promote child and family development, enhance the capabilities of young people with CP, empower their families, and chart a course into adulthood. The authors have reviewed classification and have emphasised the wide range of functional impairments that are seen in both children and adults with cerebral palsy. In doing so they have used the ICF framework and have described the range of interventions that may be applicable to people with cerebral palsy whilst emphasising that the condition is lifelong and that promotion of adaptation is a key component to understanding its nature and effects. Essential reading for parents and carers of those with cerebral palsy, clinicians, paediatricians, neurologists, occupational therapists and physiotherapists.

Описание: Each child with cerebral palsy is absolutely unique and the professionals looking after the child must provide an evaluation of his or her specific situation. In publishing this book, the editors and authors have followed a well-defined theme: even if the initial focus is orthopedics, it is aimed towards a comprehensive approach to understanding children with cerebral palsy. The reader who is aware of the orthopedic problems encountered in cerebral palsy will find the most recent data regarding various aspects of the pathology enlightening. It will also give them the opportunity to find answers to questions professionals regularly ask themselves: what are the necessary checks-ups to be done in other areas, what other measures need to be planned (whether medical, paramedical, school-related, family or socially-oriented), and how to fit all of these in with orthopedics. This book offers an approach by a group of cerebral palsy specialists who are culturally open to the worldwide community, but whom also hold on to their specific disciplines and origins as well as their differences as source of cultural wealth. This book focuses on the comprehensive approach to understanding children with cerebral palsy and beyond, to the care extended to their families, to the patients educational life as well as to their very unique being and identity.

Описание: Cerebral palsy is the most common movement disorder encountered in pediatric physical therapy practice. Physical Therapy for Children With Cerebral Palsy: An Evidence-Based Approach is a unique, comprehensive reference that focuses on physical therapy management of children with cerebral palsy through the analysis and synthesis of published research, and it offers evidence-based teaching and learning opportunities to a wide reading audience. Inside, Dr. Mary Rahlin examines the current approach to the diagnosis and classification of cerebral palsy and explores the research evidence related to prognosis; medical management; and physical therapy examination, evaluation, and intervention for children with this condition. Physical Therapy for Children With Cerebral Palsy analyzes cerebral palsy as a lifespan condition and utilizes the framework of International Classification of Functioning, Disability and Health (ICF). Sections include: •Typical and atypical development of movement and postural control•Cerebral palsy as a development disorder•Medical management of alterations of body structures and functions by body systems•Physical therapy management, including theoretical foundation, research evidence, and practice•Normal and abnormal gait patterns and current evidence for orthotic management and assistive technology•Transition to adult life Unique topics discussed: •Variability, complexity, and adaptability in motor development•The interdisciplinary team and effective interprofessional collaboration•Assessment and management of therapy-related behavior•Complementary and alternative interventions•Segmental kinematic approach to orthotic management via ankle-foot-orthosis/footwear combinationOther unique features include “Questions to Ponder” and “Suggested Questions for Future Research” at the end of each chapter. These are intended to generate healthy professional debate on a variety of topics, both in the classroom and in the clinic, and challenge the readers to plan new studies in search for evidence that will continue moving the pediatric physical therapy practice forward. Also included with Physical Therapy for Children With Cerebral Palsy is online access to video clips that accompany the text and highlight typical and atypical development, use of assistive technology, life span issues, and transition to adulthood. Instructors in educational settings can visit www.efacultylounge.com for additional materials to be used for teaching in the classroom.Physical Therapy for Children With Cerebral Palsy: An Evidence-Based Approach is intended for physical therapy students, educators, residents, and experienced clinicians, including physical therapists, other members of the interdisciplinary team, and researchers working with children with cerebral palsy.

Описание: Cerebral palsy presents stable encephalopathy that systemically influences on the childs and adults organism. Orthopedic complications of cerebral palsy possess on the contrary progressive features and those complications cause gradual failure of motor ability of a person: impaired walking, loss of postural ability, reduced self-care and inclusion, finally quality of life becoming much worse. That is why orthopedic aspects are of great importance in children with cerebral palsy. This volume presents a collection of chapters covering a wide range of problems, from a system of universal classification of dysfunctions in Cerebral Palsy and practical issues of prevention of orthopedic complications orthopedic to surgery (diagnostics and quantitative interpretation of gait disorders in such patients, optimal methods of surgical intervention, hip surgery, spinal surgery) and aspects of nutritive status of this specific category of patients, associated comorbidities and their influence and connection with orthopedic problems. Furthermore, three chapters are dedicated to the anesthetic aspects of orthopedic surgery. The substantiation is given to the methods of combined anesthesia and sedation during surgery, as well as in the immediate postoperative period, taking into account concomitant epilepsy, increased intracranial pressure and spasticity in such children. Single-event multilevel orthopedic surgery is a recognized gold standard in ambulatory patients, though it varies according to gait patterns and associated orthopedic problems causing motor disorders. The book contains explanations and methodology of surgeries for conventional gait patterns, as well as in iatrogenic disorders caused by previous unreasonable interventions. The chapter on the correction of deformities and dysfunctions of the upper limb shows the first applied transfer of m.flexor cubitus ulnaris to the radius, which gave extremely positive results both in terms of function and aesthetics. Unfortunately, in Russia the number of CP patients with iatrogenic orthopedic disorders is high. Extremely negative consequences in the form of weakness of the triceps muscles after percutaneous fibromyotomies (so-called "Ulzibat method") or lengthening of the Achilles tendon in diplegia performed before the age of five years and before the development of muscle retraction lead to the development of crouch gait even in patients with high motor potential (GMFCS I, II). This complication of unreasonable surgical interventions develops in 5-7 years after primary surgery and in the opinion of doctors and parents is not related to previous procedures. This book, based on 3D-instrumented gait analysis, shows the features of iatrogenic crouch gait, as well as methods of surgical correction of this difficult situation. The data presented in this book are based, on the one hand, on the personal practical and scientific experience of each author, and on the other hand, the entire team of authors works in collaboration with each other, coordinating their decisions and interventions with colleagues, thus ensuring continuity in treatment of children and adults suffering from cerebral palsy.
