Описание: Active infective endocarditis is one of the most serious diseases of the heart. Infection often causes periannular abscess, and may also spread to affect the mitral valve structures. This complication carries a high mortality rate and valve replacement followed by intensive care and antibiotic treatment may be the only option to save patients’ lives. Severe hemodynamic depression caused by bacterial shock, is today no longer a counter-indication, and the second major risk factor, that of postoperative reinfection, has been greatly reduced by using homografts and new valve prostheses that are resistant to bacterial infection. Timely operation, before complex destruction of cardiac structures occurs, achieves a much higher rate of survival. Active infective endocarditis is one of the most serious diseases of the heart. Infection often causes periannular abscess, and may also spread to affect the mitral valve structures. This assessment of the results of surgical treatment of active infective endocarditis was performed at the German Heart Institute Berlin. It focuses on preoperative investigation and proposes a new classification of this endocarditic disease.

Описание: Infective endocarditis has a mortality of 25-30 per cent despite advances in treatment. This could be significantly reduced by prompt diagnosis, accurate assessment and rapid institution of appropriate treatment. This is especially true of prosthetic valve endocarditis. Because symptoms are often non-specific, diagnosis is delayed for up to several weeks, sometimes with disastrous consequences. Diagnosis and assessment involve much more than simply identifying an infecting organism. It requires knowledge of valve(s) affected and of the extent of valvular damage - complications such as abscess formation, emboli and cardiac failure have to be evaluated. Without this information, the timing of correct treatment is impossible. Investigative techniques such as transoesophageal echocardiography and modern radiological procedures, for example C-T scanning, have made these tasks easier. This atlas presents the diagnostic techniques used in patients with infective endocarditis - clinical examination, microbiology, radiology and echocardiography. It outlines the steps in diagnosis, and describes the maangement decision-making process.
